In my case, and with many people who have interstitial cystitis, the citric acid breaks down the lining of the bladder and makes it extremely sensitive. Keeping the urinary pH above 6.5 allows the bladder to heal, but if I ingest citric acid in a large enough quantity (it doesn't have to be much), I will get a flare anyway, even if my bladder is nice and healthy. They've also shown that it releases histamines in the bladder, they did studies specifically with citric acid.

Before they named it "interstitial cystitis", they used to call it "allergic bladder". Sometimes I think that's a more appropriate name.

Someone said recently that IC was related to candida, but they haven't proven that. I have a friend who was cured of it by taking nystatin power for 6 months, I did the same thing and it did nothing for me. Nothing helped until I discovered the pH connection. I personally think there are probably several different kinds of IC, all with similar symptoms.
