if you are talking in terms of children who have crowded teeth and have premolars extracted and orthodontic treatment to align their teeth then crowded teeth are harder to clean and have an increased risk of decay and therefore potential loss.
Extraction and alignment of the teeth makes the teeth easier to clean and with effective cleaning can be kept for life.
With those who loose teeth in later life due to decay or periodontal disease generally all the teeth are pretty much affected the same /near abouts the same, so having the extra premolar really wont make any difference in terms of tooth loss / need for a denture
