I don't know if whatever you list is the distinguishing factor between fast foods and other foods, as you can find many if not all of these ingreadients in other foods as well.

Only way to avoid all of them is reading labels, buy organic, and cook EVERYTHING from scratch.

Antibiotics have been stopped (legally) in the meat industry because of increasing microbial resistance, as far as I am informed. The only way to realyl know is to witness it with your own eyes, or observe a lab test.
