But I need current data about You before I reply.
SO-what's your current Status? Tell me how many natural teeth are in your Head. Use the Teeth numbers when telling me exactly which teeth have fillings and which are missing, etc. Tell me the condition of your teeth by their numbers. Do this by starting at Tooth #2 and listing each tooth, in order, that has had work done in it.

[ I'm assuming you've had #1 & #16 & #17 & #32 Extracted. Tell me the extraction dates. }

List the type of work {ie., Fillings, Root Canals, etc.}, tooth by tooth in order. Include the approximate date of each repair, extraction, etc.
If you have Fixed Bridges, tell me the # of units and the # of the teeth {For Example: "I have a 3 Unit Fixed Bridge on #3-#5} and the date received }.

Next, list the State of your Gums {use the Words of your Dental HHygienist}.

Then, list How you clean your Teeth {Electric Toothbrush, the Brand, or Manual Toothbrush, etc.}. Include what You use on the Brush. Use Fluoride?
How often do You Floss?
