Sounds like my story. I still get the hormonal variety, and I also know what you mean about getting them with ovulation, but they are getting progressively less intense and less frequent since I started flushing a year ago. I took a break for about six months over the winter, but then did a few more starting this spring (doing another one tonight). After my last one, where I really got out some big ones, I only had one major headache with my period, which still wasn't as sickeningly intense as they usually have been, compared to the nearly week-long stretch that's typical for me. I used to take a lot of Vicodin for the headaches, but stopped that over five years ago, and I'm still trying to recover the damage that did to my health. The pharmaceuticals will ultimately make things worse, since they are hard on the liver. I can feel a direct connection between my GB, ovary, and menstrual headaches - literally like the headache is in my right ovary (which is enlarged & probably due to endometriosis), the liver/GB, and then runs up to my head. Maybe the meridian is inflamed, I don't know, but it's very interesting to feel that connection, since it gives me a better sense of how to approach it. I would say keep working on cleansing, particularly the bowel, liver & lymph, skin brushing, rebounding on a trampoline, exercise, sweating, drink lots of pure water, eat from nature, no processed foods, etc. & see if that helps. I'm also working on candida & parasites, which I think are also big players in this situation. Good luck!
