I've been taking 12 mg/day for almost three weeks.

I tried the single does followed by another single dose 2 weeks later. That didn't work.

I added albendazole twice a day a few days ago. Perhaps I have an iron constitution but I've really had no ill effects. :)

For an uncomplicated case the two single doses might work.

I've read that ivermectin doesn't work on larvae. I've also read that it does. I know I can kill them under my skin by holding a cotton pad soaked in ivermectin on the area.
