5 teaspoons sounds like an aweful lot of psyllium to me. They recommended dosage is one teaspoon twice daily. Could be possible you over did it. If you abdomen is tender to the touch as you say, I would seek medical advice. Appendicitis is very dangerous as it can burst causing a life threatening situation. Maybe consult with your primary dr. rather than er if you feel it is not an emergencuy right now. At least call the dr and see what he/she thinks.
I certainly would not do any enemas or take any psyllium until you have this straightened out. Your body is telling you something here, and I would be very careful. Personally I did an enema about 2 months ago and it messed me up good. I was nausous for weeks after and my bowel movements have been different since. Regardless of your thoughts on enemas, please do not do one while your appendix is painful, you will be asking for serious trouble as the bowel and appendix are closely tied together.
Anything can be good or bad for the body. Depends on whether it is done in moderation. Taking too much psyllium might cause other complications you did not bargain for. Especially while ill and your body is already in a weakened state. Go easy on yourself. By the inch
