1.) If this is what occurs inside the intestines, then why don't you get these soap stones every single time the flush is done? Also, why do some persons never get stones again after many flushes?

So these SOAP stones are formed by chemical reaction between Oil+lemon juice+bile+digestive enzymes etc. The reason the quantity and quality of stones changes from flush to flush because the bile and digestive enzymes factor cannot be controlled by us. We can regulate the amount of OIL and lemon juice we drink, but for whatever reason, IF for that particular flush, there is not enough bile/digestive enzymes, the saponification process may fail, resulting in NO STONES. This DOES NOT mean the liver has become clean.

How can you say ppl NEVER get stones after many flushes. If they keep flushing, they will get more. So there is no NEVER get stones stage.

 2.) Why did the Lahey Clinic (Boston) in the 1930s as well as many German medical practitioners today have so much success with this flush if it is/was a clinical falsehood?

I have ZERO idea about this clinic. Just googled it. http://en.wikipedia.org/wiki/Lahey_Clinic. I am very very skeptical about all these cures and frankly do not trust anything and everything I read. You should also have a healthy skepticism about such claims as many of them are fake and can be detrimental to your health.

3.) how do soap stones get into people's gallbladder whom have never done a flush in their life?

Who said that these SOAP stones got into ppl's GB ? These SOAP stones are formed in the intestines by a chemical reaction. Depending on flush to flush, the ingredients for this chemical reaction vary.

Even this poster (White Shark) who is a propagator of these flushes agree they are formed in Intestines.

Hanna has flushed 60+ times and still carries her "genuin gallstone". She says she is symtom free inspite of such a big stone. ( Very doubtful for me).
