What to do about dental visits

Ever since this odor (fecal) problem began a few years ago, I've avoided close contact with medical professionals as much as possible, as I'm sure most of you do. I have an abscessed tooth now and I really want to just have it pulled, but the dentist I've seen in the past two years (only in emergencies) doesn't want to see me. They are avoiding calling me back to set up an appointment.
The ironic thing is that the tooth with the abscess is under a crown that she placed only a year ago. I'm also having trouble with another crown she put in at that time. The rest of my teeth (that she never worked on) are just fine. My husband and I are fortunate to have a good dental plan through his medical insurance, and my dentist always gets paid for 100% of her work on the day it is done (she charges an arm and a leg).

My question to anyone who has dealt with this is what do you do when you need treatment and are getting the door slammed in your face. I thought about calling a dental school to see if there is someone kind soul there who would be willing to pull my sore tooth.
