We've had similar pasts in that we licked and picked our lips a lot. I went to the dentist in my teens, and they almost always applied Vaseline on my lips because of how chapped they were. It would always exasperate the problem though.
During the 6 months that I had EC, I began to develop a form of
Psoriasis or dermatitis on my skin - my face was extremely dry and grossly enough, I had dandruff on my hair, cheeks, nose, and eyebrows. This dandruff problem didn't go away until last year. I'm still unsure of how related the EC and dermatitis issues were.
You mentioned that you're trying the saliva/cocoa butter method because when you stopped licking your lips altogether, your condition worsened. It was similar in my case. Since I've been championing the saliva/cocoa butter method, I'm really curious of how your treatment turns out and I have some questions regarding it - How often are you licking your lips and applying cocoa butter? On my first day, and I know this sounds crazy, I did it 30 - 40 times; The amount of times I had to do it dramatically reduced by the end of the week, however. More importantly, are you using saliva/cocoa butter everytime you notice your lips drying out (besides when you're sleeping)? I think this was inherent to curing EC for me.
