So sorry to hear of the little guys problem! It's really rough on them and us when they're hurting.
Even if you can't get the prescription enemas anymore, you could try warm/tepid pure filtered water enemas. These along with stimulating his rectal area (like hanna suggested) may help 'train' his muscles to increase motility.
Also, you could try adding some finely chopped alfalfa sprouts to his food - just a little for such a little guy, maybe a teaspoon or so a day spread out over his meals. Not dried herbs or concentrated formulas, just a few sprouts a couple of days a week added to his other food. It's VERY important to buy organic seeds (research the company well first online!), rinse obsessively well with pure filtered water, and then soak for an hour in pure filtered water with 8 to 10 drops of Grapefruit Seed Extract (GSE / NutriBiotic). Drain and sprout. This helps remove any contaminents on the seeds/sprouts.
You mention massage - are you massaging just his belly or also massaging his meridian points? Being that blocked up is stressful and it may help to relax his entire body before and after massaging his tummy.
