You have scabies. I know because I have scabies and I am going through this RIGHT NOW. I googled "scabies coming out of my skin" and found your post.

I've had doctors tell me they don't know what I have, but there is a group of us who all have them. I slathered myself in neem oil tonight after a bath in neem leaves and then they started coming out. Feels like needles. At first there were many red ones. I've always been able to capture the little clear grains of sand, but tonight for the first time there are black things emerging everywhere I am infected - and some places I didn't think I was. Some are black specs, and some are thorn like black strings. I feel like a thousand needles are poking and stinging me.

So my advice - if you are still going through this - is get a coup[e of litres of neem oil from New Directions Aromatics (dot) com and bathe, scrub yourself and apply the neem. Get yourself a stiff drink. After an hour of this I had to. I don't know how long I can stay up going through this, but if I lie down I'll wipe some of the neem off and I want to get rid of these!!

I've been battling for a month. I used so much essential oil and dead Sea Salt that my skin became papery.I have ivermectin on order from India! But this neem is cheap and seems to be working, however horrifying the experience is. Apparently neem disrupts the hormone of the scabies and stops them from reproducing. I certainly hope so.
