Searching but no real details as to what a "preliminary test" entails. I suspect its all about symptom(s). In this particular case, the health care worker is left to self-monitor & when she noticed that she developed a mild temperature, it was when she reported it. Then went under observation.
In my view, a "preliminary test" could simply mean taking the individual's temperature for fever, as that is but one "symptom" of having the ebola virus. I would think that confirmation of having been infected would have to mean actual lab tests revealing virus contamination.
Having dealt with medical facilities in the past, alot of times assumptions are presumed factual, imo.
