Mold mold mold! Listen, I obviously can't diagnose you, but I have similar symptoms and know for a fact that fungus has colonized my nose. I NEVER had sinus or anxiety problems until being exposed. Look into natural or Rx antifungals from a well educated doctor. The reason we've tried and failed with so many different protocols, is because we are not killing the pathogens!
