New EC research website

As there appears to be a significant lack of research done on the subject of EC (I've only seen one basic data capture website, which is useless).

I've decided to build an EC research website that will ask suffers a serious of multiple choice questions about their condition and lifestyle so that I can collate all the data and look for patterns.

My aim is to find a pattern that can help identify the cause, and hopefully lead the way to a cure.

QUESTIONS NEEDED!

I need help to coming up with enough multiple choice questions about peoples lifestyle and condition so that the mass data collected is useful. Please reply with any suggestions.
